FBI & BCI - $65.00

BCI Only - $30.00

FBI Only - $35.00

Two forms of ID are required:

(1) Photo ID

and one of the following:

Social Security Card
Birth Certificate
Passport

Not sure if you need a background check (BCI/FBI fingerprinting)? 
Please answer the following questions:

1.  Have you lived in Ohio continuously for the past 5 years?
         a.  Yes.  Move on to question #2.
         b.  No.   Are your BCI and FBI less than 5 years old?
                 i.  Yes. You are good, no need to redo your fingerprints.
                ii.  No.  You will need to redo both BCI and FBI fingerprints.
                          Call for an appointment

2.  Do you have a BCI report on file with the Ohio Department of Education (ODE)?
         a.  Yes.  Move on to question #3.
         b.  No. You will need to redo your BCI fingerprints and then move on to question #3.
                          Call for an appointment

3.  Is your FBI report less than 5 years old?
         a.  Yes.  You are all set, no need to redo your prints at this time.
         b.  No.  You will need to redo your FBI fingerprints.
